Police investigating the fatal head-on collision near Kumbia on Monday night are appealing for further witnesses or anyone with dashcam footage of the incident to come forward.

A southbound car was in the northbound lanes of the Bunya Highway when it collided with a northbound truck about 7:20pm.

The force of the collision caused both vehicles to catch on fire.

A woman from Eli Waters and her four children died as a result of the crash.

Media reports have identified them as Charmaine McLeod, 35; Aaleyn, 6; Matilda, 5; Wyatt, 4; and Zaidok, 2.

The driver of the truck, a 47-year-old Taabinga man, was taken to Kingaroy Hospital for treatment of non-life-threatening injuries.

A police spokesperson said forensic testing was under way to formally identify the victims.

The Forensic Crash Unit is investigating and will prepare a report for the Coroner.
